Before Your Initial Consultation
We are genuinely looking forward to meeting you. We know that getting to your appointment takes real effort: time away from work, away from family, navigating traffic and parking. Every minute you spend with Dr. Eskew should feel worth it. A little preparation goes a long way toward making that happen. Here is what to expect and how to get ready.
SYMPTOM HISTORY
To make the most of your time together, it helps to reflect on your symptoms before you arrive. These are the questions Dr. Eskew will walk through with you, so take a moment to think about your answers ahead of time.
When did your symptoms begin? Was there an acute event or trauma that led to them?
When do you notice your symptoms? Are they constant, or do they worsen with certain activities?
Where is most of your pain located?
How would you rate your pain on a scale of 0 to 10? If your joint was completely healthy, that would be 100%. If it were completely disabled, that would be 0%. How would you rate it overall?
Over the past few months, are your symptoms getting better, staying the same, or getting worse?
What activities for work, sports, and recreation matter most to you, and how are your symptoms affecting your ability to do them?
TREATMENT HISTORY
Please bring a detailed record of any prior treatments: physical therapy, medications, injections, and any previous surgery. If you've had surgery before, request a copy of the operative report from that surgeon's office and bring it with you.
PREVIOUS IMAGING
If you've had any prior imaging (X-ray, CT, or MRI), please contact that facility and request both the report and the actual images on a CD to bring to your appointment. If you haven't had recent imaging, don't worry, we'll take care of that during your visit. If you're a new patient and haven't had recent X-rays, we will likely obtain a new set the day of your appointment.
